Benefit Details

 

Accidental Hospitalization (including day-care treatment) benefit shall be applicable as per the following:

Types of Accidental Injuries Coverage Amount
Head Injury 100% (BDT. 50,000)
Chest Injury, Internal Injury 50% (BDT. 25,000)
Fracture, Dislocation, Dismemberment, Amputation 30% (BDT. 15,000)
Scrape and Cut, Abrasion, Soft Tissue Injury 10% (BDT. 5,000)

 

CLAIM DOCUMENTATION

For Accidental Hospitalization (including day-care treatment)

  1. NID and Valid Driving License of the Covered Person
  2. Doctor’s Advice for hospitalization (in applicable cases)
  3. Discharge Certificate (in applicable cases)
  4. Hospital Bills
  5. All medical reports such as X-Ray, City Scan, MRI or such relevant reports
  6. Any other supporting documents (if required)

For COVID-19

  1. Doctor’s advice for COVID-19 Test (RT-PCR Test)
  2. COVID-19 (RT-PCR) test report stating “Positive” results following government approved guidelines
  3. Diagnostic test bill
